ABSTRACT

Background: The COVID-19 pandemic significantly impacted routine cardiovascular health assessments and services. The objective of this study was to explore the factors associated with self-reported psychological distress among a sample of patients with atrial fibrillation (AF) in China in relation to COVID-19. Methods: An online survey was administered to 288 patients with AF at several hospitals in China. The survey consisted of three sections: demographic characteristics, questions related to COVID-19, and the General Health Questionnaire-12 (GHQ-12). Results: A total of 177 patients with AF completed the baseline survey; 177 (61.46%) were male and 133 (46.18%) were older than 65 years. High levels of psychological distress (GHQ-12 ≥3) were observed in 27 (9.4%) participants of the sample. These high levels were found to be associated with older age, radiofrequency ablation, drinking, and combined basic diseases (p values < .05). Logistic regression analysis showed that psychological distress in patients with AF was associated with radiofrequency ablation (OR = 0.316, 95% CI = 0.147–0.666), drinking (OR = 4.761, 95% CI = 2.076–10.916), and concerns regarding infection (OR = 1.244, 95% CI = 1.052–1.472). Conclusions: COVID-19 has resulted in high levels of psychological distress in approximately 9.4% of patients with AF in China. Factors associated with high levels of psychological distress in AF patients include older age, radiofrequency ablation, drinking, and combined comorbidities. These findings highlight the importance of enhancing psychological health throughout the course of infectious pandemics.

ABSTRACT

OBJECTIVE Since the coronavirus disease 2019(COVID-19) outbreak in December 2019, the search for a potential treatment for COVID-19 has been a constant focus. Therefore, we identified potential treatments for COVID-19 from Hippophae Fructus, a Tibetan medicine that may act on COVID-19, using a network pharmacology approach.METHODS We collected the chemical constituents and corresponding targets of Hippophae Fructus from traditional Chinese medicine system pharmacology(TCMSP). COVID-19 related genes were predicted in pubmed-Gene, OMIM and GeneCards databases. Then, protein-protein interactions(PPIs) of key genes were analyzed by STRING database.Compound-target-diseases network was constructed using Cytoscape software. The potential pathways were determined by Gene Ontology(GO) and Kyoto Encyclopedia of Genes and Genomes(KEGG) pathway analyses. Additionally,molecular docking was used to verify the binding effect between the active component and the target. RESULTS A total of 33 components and 192 corresponding targets in Hippophae Fructus were found. 50 genes were obtained from the intersection of component targets and disease targets. These genes include IL-6, TNF, MAPK8 and PTGS2, which regulate several pathways associated with COVID-19, involving Hepatitis B, Influenza A, TNF signaling pathway and Tuberculosis. More importantly, high-node compounds such as quercetin and beta-sitosterol can well bind to key targets.CONCLUSION Some components in Hippophae Fructus can act on COVID-19 related genes and regulate multiple pathways. Perhaps Hippophae Fructus has the effect in treating COVID-19.

ABSTRACT

BACKGROUND: Celastrol has been proven effective in anti-inflammatory but was limited in the clinic due to the poor solubility and side effects induced by low bioavailability. Osteoarthritis has acidic and inflammatory environment. Our aim was to load celastrol into HMSNs and capped with chitosan to construct a pH-responsive nanoparticle medicine (CSL@HMSNs-Cs), which is of high solubility for osteoarthritis intra-articular injection treatment. METHODS: The CSL@HMSNs-Cs were assembled and the characteristics were measured. The CSL@HMSNs-Cs was applied in vitro in the chondrocytes collected from rats cartilage tissue and in vivo in the MIA induced knee osteoarthritis rats via intra-articular injection. Cytotoxicity assay, pH-responsive release, pain behavior, MRI, safranin o fast green staining, ELISA and western blot analysis were applied to evaluate the bioavailability and therapeutic effect of CSL@HMSNs-Cs. RESULTS: CSL@HMSNs-Cs was stable due to the protection of the chitosan layers in alkaline environment (pH = 7.7) but revealed good solubility and therapeutic effect in acidic environment (pH = 6.0). The cytotoxicity assay showed no cytotoxicity at relatively low concentration (200 µg/mL) and the cell viability of chondrocytes stimulated by IL-1ß was increased in CSL@HMSNs-Cs group. Paw withdrawal threshold in CSL@HMSNs-Cs group is increased, and MRI and Safranin O Fast Green staining showed improvements in articular surface erosion and joint effusion. The upregulated expression levels of IL-1ß, TNF-α, IL-6, MMP-3 and MMP-13 and NF-κB signaling pathway of chondrocytes were inhibited in CSL@HMSNs-Cs group. CONCLUSION: Hollow mesoporous silica nanoparticles were an ideal carrier for natural drugs with poor solubility and were of high biocompatibility for intra-articular injection. These intra-articular injectable CSL@HMSNs-Cs with improved solubility, present a pH-responsive therapeutic strategy against osteoarthritis.

ABSTRACT

INTRODUCTION: The COVID-19 pandemic presents a unique global health challenge further complicating surgical management of COVID-19 positive patients due to a lack of published literature. CASE: Within we discuss a 48-year-old Chinese man, presenting with acute gastrointestinal obstruction due to sigmoid colonic mass. The patient was screened and tested positive for COVID 19 due to his employment in Wuhan, China at the COVID-19 pandemic epicenter. The patient was subsequently taken for open sigmoid colonic resection, however the case presented multiple challenges due to the patient's COVID-19 positive status. DISCUSSION: The challenges of surgical management of COVID-19 positive patients exist are four-fold. First the unknown efficacy of pre-surgical risk stratification in COVID-19 positive patients, second the risk of aerosolized COVID-19 transmission during intubation for surgery, third the risk of fecal COVID-19 transmission to surgical staff during large bowel resection, and fourth the post-operative challenges of caring for COVID-19 positive patients. CONCLUSION: Further research is needed into these topics, as well as the medical management of COVID-19 surgical patients.

ABSTRACT

1. BackgroundTo explore the manifestations and evolution of the pulmonary CT in COVID-19, and to analyze the causes and countermeasures of “Recurrent positive” in discharged patients.2. MethodsData of 39 patients with COVID-19 were collected. RT-PCR was positive at admission.From onset to discharge, pulmonary CT was performed regularly.During the treatment,Blood-RT,CRP and D-dimer were detected.3.ResultFrom the onset to 14 days, the lesions in pulmonary CT increased significantly.After treatment, pulmonary CT before discharge showed that some patients' lesions were completely absorbed, and some residual strip like lesions or ground glass opacity with reduced density.Two weeks after discharge, there were 2 patients with new ground glass opacity.There were 20 patients with D-dimer increased.4.ConclusionIn the early stage of COVID-19, the pulmonary CT has the characteristic manifestations, which is helpful for early diagnosis.In the middle stage, pulmonary lesions changed rapidly.In the recovery stage, some of the patients remained strip like lesions.It is necessary to pay attention to the possibility of pulmonary fibrosis after recovery.The discharge standard of COVID-19 needs to be more strict to avoid “Recurrent positive”,the discharged patients should continue to be observed.D-dimer was increased in some patients, it is safe to use heparin in anticoagulation without contraindications.

ABSTRACT

Objective@#New infectious diseases have become a global problem that seriously threatens human life and health and social development, which greatly increases the demand of CT examination for outpatients with fever. Through the discussion of the examination room design of the special CT for epidemic prevention, it can provide reference for medical institutions and reduce cross infection.@*Methods@#Based on the requirements of CT equipment installation and environment, combined with the special requirements of epidemic prevention in fever clinic, the paper analyzed the location of examination room, room layout, site construction method, air purification and disinfection, intelligent image aided diagnosis of special CT for epidemic prevention, and put forward the design scheme.@*Results@#Through the detailed analysis of the key points of engineering technology and the requirements of infection prevention and control of CT examination room, the design scheme of the examination room was given.@*Conclusions@#The establishment of special CT for epidemic prevention can meet the needs of clinical examination and effectively reduce cross infection. The design scheme given in this paper has certain reference value and can provide effective help for medical institution.

ABSTRACT

Background: The dynamic changes of lymphocyte subsets and cytokines profiles of patients with novel coronavirus disease (COVID-19) and their correlation with the disease severity remain unclear. Method: Peripheral blood samples were longitudinally collected from 40 confirmed COVID-19 patients and examined for lymphocyte subsets by flow cytometry and cytokine profiles by specific immunoassays. Findings: Of the 40 COVID-19 patients enrolled, 13 severe cases showed significant and sustained decreases in lymphocyte counts but increases in neutrophil counts than 27 mild cases. Further analysis demonstrated significant decreases in the counts of T cells, especially CD8 + T cells, as well as increases in IL-6, IL-10, IL-2 and IFN-{gamma} levels in the peripheral blood in the severe cases compared to those in the mild cases. T cell counts and cytokine levels in severe COVID-19 patients who survived the disease gradually recovered at later time points to levels that were comparable to those of the mild cases. Moreover, the neutrophil-to-CD8+ T cell ratio (N8R) were identified as the most powerful prognostic factor affecting the prognosis for severe COVID-19. Conclusion: The degree of lymphopenia and a proinflammatory cytokine storm is higher in severe COVID-19 patients than in mild cases, and is associated with the disease severity. N8R may serve as a useful prognostic factor for early identification of severe COVID-19 cases.
